Application Form for the Data Subject
(Within the Scope of Law No. 6698 on the Protection of Personal Data)
Pursuant to Law No. 6698 on the Protection of Personal Data (“Law”), natural persons whose personal data are processed (hereinafter referred to as the “Applicant”) are granted the right to make certain requests regarding the processing of their personal data, as outlined in Article 11 of the Law.
In accordance with Article 13, Paragraph 1 of the Law and the “Regulation on the Procedures and Principles of Application to the Data Controller,” applications must be submitted to Elit Tur via one of the methods listed below, either in writing or through electronic means.

Application Methods
According to Article 5/2 of the Regulation dated 10.03.2018 (published in Official Gazette No. 30356), the following information must be included in applications regarding personal data protection:
•Full Name
•Signature (for written applications)
•Turkish citizens: T.R. Identity Number
•Foreign citizens: Nationality, passport number or (if available) ID number
•Residential or business address
•Email address (if available, for communication)
•Telephone number, and fax number if applicable
•Subject of the request

1 – Written Application:
A fully completed and hand-signed copy of this form must be submitted:
•In person
•By a representative holding a notarized power of attorney
•Or via notary public

•Via email, sent from the email address previously registered and stored in Elit Tur’s system by the data subject
•Through a designated software or application developed for this purpose (Currently, Elit Tur does not offer such an application)

Requests will be processed within 30 days from the date of receipt, as stipulated in Article 13, Paragraph 2 of the Law, and responses will be provided in writing or electronically, depending on your preference.

The official date of application is the date your request is received by us. Incomplete applications will be rejected. In order to protect your personal data and prevent unauthorized disclosure, we reserve the right to request additional documents for identity and authorization verification. You will be notified if such documentation is required.
Elit Tur will not be held responsible for any claims arising from inaccurate or outdated information provided in your application or from applications submitted by unauthorized individuals.
Your personal data submitted through this form will be stored and deleted in compliance with applicable technical and organizational measures after the legally required retention period.
If the processing of your request incurs additional costs, you may be charged fees in accordance with Article 7 of the above-mentioned Regulation.
